Essential care partners provide physical, psychological, and emotional support, as identified important by the patient/ resident. This care can include support in decision making, care coordination and continuity of care. Essential care partners are identified by the patient/ resident (or substitute decision maker) and can include family members, close friends, or other caregivers.
The Essential Care Partner (ECP) Support Hub, led by the Ontario Caregiver Organization and in partnership with Ontario Health aims for a consistent identification, inclusion, and support for family caregivers. The Support Hub provides guidance and tools to healthcare settings, supporting with the implementation or enhancement of Essential Care Partner programs and practices.
Examples of different Essential Care Partner program practices are:
Caregiver identification, such as enabling patients to identify an essential care partner during the admission/registration process and providing a caregiver ID badge.
Caregiver inclusion, such as developing shared understanding of mutual expectations of care providers and care partners in care setting policies, and formalizing roles for caregivers when the care team works together.
Caregiver support, such as caregiver orientation, education and training, on-site support services, and linking caregivers to support for their own well-being.
We are looking for caregiver input on resources we are developing to help communicate the important role of essential care partners in healthcare settings.
